Restaurants with some of the highest numbers of “risk factors,” which inspectors consider major violations of health codes, are selected for our Dirty Dining Report. Here are the restaurants in the Valley that did not make the grade on November 2, 2022:
Papa John’s Pizza – 1813 E. Southern Ave, Tempe
3 violations
- An employee who handles ready-to-eat foods with bare hands
- The walk-in refrigerator does not keep the food cold enough
Giordano’s Trattoria Romana – 7275 E. Easy, Carefree St
3 violations
- The marinara sauce was left out
- Cooked meat is not kept at the correct temperature
Burger Shoppe – 3423 W. Thomas Road, Phoenix
3 violations
- Chicken strips stored in grocery bags
- Breakfast burritos not kept warm enough
Kimthao Bakery – 2050 N. Alma School Road, Chandler
4 violations
- Cooked shrimp, chicken, and pork are not properly cooled
- Flana in the refrigerator without a date mark
Kyoto bowl – 1245 W. Elliot Road, Tempe
4 violations
- White, fuzzy, organic growth on green peppers and carrots
- Pest control powder for washing hands under the sink
- Sauce bottles in the fridge with the top cracked

Carlos O’Brien’s Mexican Restaurant – 7000 E. Mayo Blvd., Phoenix
US Egg – 7119 E. Shea Blvd, Scottsdale
